Stress and anxiety are often thought of as purely mental or emotional experiences—but their impact goes far deeper. One of the most affected systems in the body is the digestive tract. The gut–brain axis: A two-way conversation

At the heart of this relationship is the gut–brain axis, a complex communication network linking the brain and digestive system. This system allows signals to travel in both directions via nerves. This means emotional states like stress and anxiety can directly influence digestion, while gut disturbances can, in turn, affect mood and mental wellbeing.

How stress and anxiety affect digestion

1. Disruption of gut motility

When you’re stressed, your body activates the fight-or-flight response, releasing stress hormones like cortisol. These hormones alter the movement of the digestive tract (gut motility). Some people experience slowed digestion, leading to constipation and bloating. Others experience accelerated digestion, causing diarrhoea. This explains why stress often leads to unpredictable bowel habits.

2. Changes to the gut microbiome

The gut contains trillions of bacteria that play a key role in digestion, immunity, and even mental health. Chronic stress can disrupt this delicate balance—a condition known as dysbiosis. An unhealthy microbiome can

•Impair digestion

•Influence mood and anxiety levels

There is growing evidence that changes in gut bacteria are linked to mental health conditions such as anxiety and depression.

3. Increased gut sensitivity and pain

Stress and anxiety can lower pain thresholds in the gut, making normal digestive processes feel uncomfortable or painful. This is particularly evident in conditions like irritable bowel syndrome (IBS), where stress often triggers flare-ups of symptoms such as abdominal pain, bloating, and irregular bowel movements.

4. Gut inflammation and the “leaky gut”

Chronic stress can weaken the intestinal lining, making it more permeable—a condition commonly referred to as “leaky gut.” This allows unwanted substances (like toxins and bacteria) to enter the bloodstream, triggering inflammation and potentially contributing to wider health issues.

5. Changes in neurotransmitters

The gut plays a surprisingly large role in producing neurotransmitters. In fact, most of the body’s serotonin—a key mood regulator—is made in the digestive tract. When stress disrupts gut function, it can alter these chemical signals, potentially worsening anxiety and low mood. This creates a feedback loop where anxiety affects the gut, and gut issues worsen anxiety.

Breaking the cycle

The good news is that improving one side of the gut–brain connection often benefits the other. Strategies include:

•Stress management (mindfulness, therapy, relaxation techniques)

•Balanced diet rich in fibre and fermented foods

•Regular exercise

•Increasing water intake
